Kendal-Crosslands ,The provider of quality senior living, is currently seeking Full-Time Social Worker to join our team in Kennett Square, PA. We offer Independent Living cottages and apartments, Personal Care, and Skilled Nursing all nestled on over 500 acres of rolling country-side in the heart of the Brandywine Valley.Job Type: Full-timeFull Job Description: The primary purpose of the Social Worker is to address the psycho-social needs of residents by promoting independence self determination and personal responsibility in a manner which is consistent with Kendal-Crosslands communities philosophy.Duties/Responsibilities:

  • PROVIDES COUNSELING AND OTHER SUPPORT SERVICES, TO APARTMENT AND HEALTH CENTER RESIDENTS BY:
  • Assessing new health center residents to identify their psycho social needs.
  • Working with residents who desire intermittent or on-going counseling or psychotherapy
  • Acting as a problem solver by assisting families and residents to find appropriate resources
  • providing referrals and resources to individual residents
  • Assisting residents in decision making relative to transfers to different levels of care.
  • Providing discharge planning for residents returning to their apartments or home outside the community.
  • Maintain contact with families, friends and significant others regarding social, emotional and medical problems.
  • Completes and maintains all required documentation within specified time frames
  • Ability to conduct Teleconference (Zoom, WebEx) technology for communication ease with care conference with POA/HC/ families and for resident education meetings as needed.
    • SUPPORTS KENDAL-CROSSLANDS AND THE SOCIAL SERVICE DEPARTMENT BY:
    • Designing and participating in in-service training programs relating to the social and emotional needs of the elderly as appropriate. Using in-services and other opportunities to help staff recognize the problems and needs of the elderly.
    • Developing programs for the community that meet the needs of various resident groups.
    • Interviewing prospective residents and participating in the admissions process.
    • Keeping abreast of advances in gerontology and social work through continuing education,lectures, seminars and in-service training.
    • Attending and participating in meetings as appropriate including, but not limited to: Care Conference, Resident Care/Move Meeting and Kendal-Crosslands Social Services Department Meetings.
    • Supports field placement experience (bachelor's and master's levels) as part of the teaching and educational role of the Kendal-Crosslands Communities.
    • Supporting staff members in their efforts to meet psychosocial needs of residents.
    • Educational understandings related to cognitive decline, Alzheimer's as well as the emotional sensitivities to decrease stigma associated with a dementia diagnosis. To act as a role model in interactions and offer intervention ideas in bi-monthly behavioral reviews with staff. To participate in dementia education planning for residents and staff.Qualifications: A Master's degree in Social Work, or related field plus a minimum of 2 years experience, preferably in a geriatric setting. Preferred candidate has a current PA license LSW,or be eligible for licensure.Physical Demands:
      • Requires the mobility to visit residents and/or staff in various locations and may be required to drive a car to make visits with residents.
