Job Description:
ROLE AND RESPONSIBILITIES
The duties listed below are intended as examples of the types of
work that may be performed by a Case Manager for the Community
Reintegration Services Program (CRSP). but are not all necessarily
performed and do not include all specific functions and
responsibilities that the Case Manager may be expected to
perform.
--- Provide in-depth, wrap-around case management services to a
caseload of individual clients referred through the Probation
Department who have recently exited prison and are currently in the
community on probation.
--- Perform needs assessment with each client, create appropriate
case plans, and conduct individual case management sessions with
clients, both at intake and at regular intervals while cases remain
active.
--- Utilize a variety of interventions such as TCU Mapping,
Cognitive Behavioral Therapy, and Motivational Interviewing to
support client wellbeing and target criminogenic risk factors.
--- Coordinate services and actively place clients at relevant job
training programs, housing sites/programs, employment
sites/programs, and social service, substance abuse and mental
health treatment programs.
--- Assist in client's resource utilization by accompanying or
transporting clients to off-site programs, jobsites, and social
service providers.
--- Provide frequent client follow-up contact and support via
phone, off-site visits, or office visits.--- Assist with workforce
development and employment lab at Probation & Parole locations.
--- Assist participants with building skills to manage daily
stressors, improve impulse control and reduce negative
behaviors.
--- Communicate with the courts and probation and parole and
advocate during court proceedings when needed; and maintain and
foster relationships with community partners.--- Assist with
creating program marketing materials and digital infographics.
--- Serve as a program representative as needed at community
meetings.
--- Assess and report on client progress and participation at
regular intervals.
--- Track and update client information using the agency's database
program.
--- Generate monthly, quarterly reports and year-end reports to
Dept. of Corrections and Management.
--- Coordinate, re-stock, and maintain documents for on-site food
pantry.
--- Accept and perform other work as assigned.
QUALIFICATIONS AND EDUCATION REQUIREMENTS
--- Significant knowledge and experience related to
justice-involved people, the social forces and systems impacting
them, and the resources available to them required. Bachelor's
Degree in human services, social work, or a related field also
required. Master's degree preferred. Experience in motivational
interviewing, strengths-based case management, and/or Cognitive
Behavioral practices also preferred.
--- Background checks and clearances required as per program
contract(s).
--- Basic computer skills, including but not limited to the use of
Microsoft Office, as well as Zoom/Skype and other distance meeting
resources.
--- Ability to multitask and work both independently and with
supervision, on-site and remotely.
--- Embrace an understanding of justice involvement that
acknowledges the impact of institutional racism and the systemic
oppression of BIPOC, women, and LGBTQ+ people; must share the
belief that people can change in positive ways.
--- Must embrace teamwork, have a strong work ethic, be a
self-starter with a positive attitude, with a good sense of
humor.
--- Strong team building, organizing, writing and other
communication skills are essential.
