FUNCTION:Provides day-to-day direction and support of the chart and
administrative form management staff and processes including
analysis, design, development and implementation of form solutions.
- Assesses medical record documentation to verify compliance with
regulatory requirements. -PRINCIPAL DUTIES AND RESPONSIBILTIES:
- Supports the creation, distribution, interaction with, and
integration of medical record forms within the organization. -
- Controls and monitors the flow of forms development work. -
Manages ongoing form review to meet forms management policy. -
Prioritizes work to meet timelines and business needs.
- Makes recommendations on the strategic direction of
documentation tools in terms of technology, integration and
content.
- Suggests and implements best practices for forms management
including the adoption of electronic forms, integration with
electronic systems, and regularly gathers data and analytics to
recommend new technology and products.
- Designs, tests and implements forms for electronic signature.

- Provides oversight to system access and security, data capture,
and information workflow.
- Conducts medical record reviews to validate inpatient and
outpatient surgery documentation compliance with Joint Commission
and other regulatory agency standards. - Coordinates provider
hands-on medical record review process and produces detailed
outcomes report.
- Works with departments throughout the organization to improve
documentation compliance. Trouble shoots root cause of
documentation issues, makes recommendations for documentation
improvement and drives organization to implement improvement
opportunities. -
- Prepare and revise policies and procedures as warranted. -
Manage tools such as information flowcharts, instructional manuals,
and forms in order to promote effective use of applications. -
Provide documentation and training for users when there is a system
change or update.
- Provide support and assistance to users and ongoing education
and training to all staff including HIM staff, clinicians and
others.
- Coordinate system downtime related to maintenance and
communicate with affected users and managers.EDUCATION AND
EXPERIENCE REQUIREMENTS:
- Bachelor or Associate Degree in Health Information Management
or other field (Bachelor Degree with RHIA or RHIT). -
- Minimum of two years of experience in HIM or related field with
a focus on information technology support.
- Experience documenting processes to include flowcharting
required.KNOWLEDGE, SKILL, AND ABILITY REQUIREMENTS:
- Thorough knowledge of medical record content requirements.
- Thorough understanding of comprehensive inpatient and
outpatient clinical documentation standards with knowledge of
electronic health record applications.
- Maintain a high level of understanding of HIPAA, Joint
Commission, state and federal regulations and
- guidelines, hospital policies and procedures, and AHIMA best
practice methods regarding maintenance of
- health records and health information.
- Understanding of system architecture, infrastructure,
integration, and databases.
- Basic understanding of database design and data management
concepts and models.
- Strong trouble shooting and communication skills used for both
implementation and support of the HIM applications.
- Knowledge of current EHR and HIM-related electronic systems and
technologies.
- Demonstrated leadership skills with the ability to create a
collaborative working environment, solve problems, handle
conflicts, and influence groups to work toward a common goal.
- Strong verbal and written communication skills, including
active listening -and effective expression of facts, ideas and
opinions in discussions, meetings, reports and presentations.
- Strong organizational skills, including the ability to assemble
and analyze large volumes of information (e.g., during requirements
gathering) and manage multiple assignments with unexpected shifts
in priority.
- Ability to evaluate and document business requirements and work
processes including flowcharting.
- Ability to work independently with minimal supervision. -
- Basic understanding of system architecture, infrastructure,
integration, and database concepts.
