range from $25.00 to $27.60 per hour depending on experience.
Essential duties for this position: Plan and deliver direct care to
residents as allocated under the responsibility of this job. You
will organize, oversee, and carry out shift schedules for the
residents of the designated nursing unit. This position offers the
following full time shifts: Day shift, night shift, and overnight
shifts Quality of Care: Completes position-specific key functions
and responsibilities in a timely manner with accuracy that have an
impact on the overall quality of care. Encourages open
communication with the management team, direct care staff,
residents, and families to guarantee that residents receive the
best care possible. Basic knowledge of the five-star rating system
and how it relates to the duties and obligations unique to each
position. Complies with all local laws and regulations regarding
the investigation, reporting, and prevention of abuse of vulnerable
adults. KEY POSITION SPECIFIC ROLES & RESPONSIBILITIES Clinical
Processes & Procedures Ensure that nursing notes are informative
and complete regarding the care and services provided to the
resident. Discuss resident diagnoses and treatment plans openly
with the Medical Director and DON. Create a treatment plan in
collaboration with the clinical team. Makes walking rounds between
shifts and whenever necessary to supervise and monitor work
performance in terms of resident care, staff regulations, and
efficiency of service. Complete all documentation as recommended by
Federal and State Guidelines. Ensure that reportable events are
communicated in a timely fashion. Unit Management Maintain open
lines of communication with your supervisor, your coworkers, and
the upcoming shift while updating all shift reports. When
necessary, conduct physical and mental nursing evaluations on
residents. Performs other duties as assigned. MINIMUM JOB
REQUIREMENTS Education Successful completion of a Licensed Practice
Nurse (L.P.N.) program at an accredited school, college, or
university. Experience Previous experience as an L.P.N. in
long-term care preferred. Must have and maintain an active L.P.N.
license/registration in good standing on the state registry. CPR
certified. Availability & Attendance Flexible work schedule
(mornings, afternoons, nights, weekends, holidays, etc.) which may
include hours beyond eight hours in a day, or 40 hours in a week.
Follows all attendance policies and procedures, including
punctuality and proper time clock or timesheet reporting.
Compliance with infectious disease policies and procedures.
