St. Martha Villa has opportunities for capable and motivated individuals to join our team as an LPN Supervisor.
The LPN Supervisor provides effective and efficient nursing care while achieving positive clinical results and resident/family satisfaction. The LPN Supervisor complies with the policies of practice defined by the State Nurse Practice Act and delegates responsibilities in resident care to CNA's consistent with their capacity of practice.
LPN Supervisor Responsibilities:

• 
  
• Manages resident care through delegation of care responsibilities to staff regarding, complexity of care, competency of staff, facility policies, and the Pennsylvania Department of Health regulations and Federal Health Care Administration
  
• Supervises HHAs to ensure that delegated tasks and interventions are completed appropriately and per policyEvaluates and adjusts the plan of care to achieve resident goals and minimize re-hospitalizations
  
• Assist residents with medications when assigned.
  
• Participate in monthly changeover of MARs/TARs and assist with planning for adequate staffing to ensure coverage for this process.
  
• Responsible for assisting with finding shift coverage when required.
  
• Oversee shift change and participate as necessary with resident aides/HHAs in shift-to shift reporting.
  
• May participate in treatment team if working day shift.
  
• Maintain timely communication with nursing, case managers, physicians and families regarding pertinent resident issues and changes in condition.
  
• Document accurately and timely in resident progress notes and shift-to-shift report.
  
• Assist with auditing medical evaluation forms, and assist with admission paperwork
  
  as assigned.

  • Document timely and appropriately in resident's individualized service plans.
    
  • May conduct quality assurance audits of residents who self-medicate including those who administer own blood glucose monitoring and insulin pens.
    
  • Ensure other audits are completed as assigned and as relevant to your shift.
    
    LPN Supervisor Qualifications:
    
    • 
      
    • Graduate of an approved School of Nursing is required
      
    • Must have and LPN license by the State Board of Nursing
