patient as indicated by changes in condition and response to
treatment. Coordinates patient, family, significant other, and
health team members in the planning of hospital and post-discharge
care. Applies teaching and learning principles in the education of
patients and families. Identifies unit, staff, and patient care
problems. Takes action or refers problems to appropriate resources
when indicated. Coordinates patient activities with other
