Job Description Direct Hire – RN Care Manager Schedule:
Full-Time | Mon–Fri | 9:00 AM – 5:00 PM | Weekend rotation (1 Sat &
Sun per month) Salary: $126,160 – $160,090 Benefits: Full benefits
package retirement plan Position Overview Seeking an experienced RN
Care Manager to perform comprehensive patient assessments,
coordinate multidisciplinary care, and support safe discharge
planning. The role focuses on identifying high-risk patients,
preventing readmissions, and ensuring appropriate utilization of
healthcare services while collaborating with physicians and the
care team. Key Responsibilities Conduct initial and ongoing patient
assessments Identify high-risk patients and coordinate supportive
services Manage care coordination and discharge planning Perform
Utilization Review (UR) activities and payer reviews Arrange
post-discharge care and patient education Collaborate with
healthcare teams to ensure safe patient transitions Requirements
BSN required Active NY RN License 3 years clinical experience (ED,
Critical Care, or Med/Surg preferred) PRI Certification (within 3
months of hire) Strong computer and documentation skills Preferred:
Screen Certification, Case/Care Management certification (CCM/CMC),
prior case management experience, and familiarity with
