Job Description:
ChristianaCare is currently seeking a full-time Registered Nurse
for our Holloway Community Program. In this role, you will be
responsible for facilitating and coordinating care of patients
(pediatric, adolescent, adult, & geriatric) with HIV, HCV, and
other infectious diseases with a predominantly sexual mode of
transmission, and for HIV prevention. This role also serves as a
resource for ChristianaCare Health System caregivers, as well as a
liaison to community partners. Education and counseling to patients
and families related to health care and disease management is also
a priority. The home base for this position is located on the 6th
floor of the Gateway Building, Wilmington campus. Also, may require
Monday hours at Beautiful Gate on Walnut Street in Wilmington and
Tuesday morning hours at Brandywine Counseling on Lancaster Avenue
in Wilmington. Some travel between sites including Smyrna and
Milford may be needed. Will discuss during HR interview. This is a
grant funded position. PRINCIPAL DUTIES AND RESPONSIBILITIES:
Functions as a role model and resource person in the clinical
setting; demonstrates competency in clinical judgment and a strong
knowledge base of current clinical practice. Demonstrates skills
and knowledge necessary to provide care appropriate to pediatric,
adolescent, adult, and geriatric patients, including knowledge of
growth and development, the ability to obtain and interpret
information to identify patient need, and to provide the care
needed. Assesses the physical and psychosocial health status of
patients through the collection of health data. Plans and initiates
appropriate interventions within scope of practice and evaluates
effectiveness. Responds to patient request with promptness,
empathy, and genuine concern as perceived by patients, community
partners, and other ChristianaCare caregivers. Provides discussion
and nursing interventions for problematic patient issues.
Collaborates with members of the health team to provide support
services that facilitate the delivery of patient care. Consults
with providers and other health team members when appropriate.
Assures appropriate patient follow-up in the outpatient setting,
coordinates with community partners and professional outpatient
services. Coordinates discharge planning and follow-up of the plan
of care. Develops and maintains effective communication within the
health care team and other departments. Participates in quality
assurance activities and implementation of nursing standards
Evaluates to establish teaching/learning priorities for patients
and facilitates implementation; assists in the development of
learning materials to facilitate the teaching process for patients.
Continuously evaluates and revises methods of teaching based on the
needs of a changing patient population Provides and coordinates
education for patients and families in the areas of disease
process, treatment modalities, and health maintenance. Selects and
adapts teaching models and methods based on identified learning
needs of specific patient population. Utilizes Teach Back
methodology to ensure understanding for patients and their
families. Assures patients are properly informed of treatment
modalities and services provided within the Wm. J. Holloway
Community Program. Assists providers with procedures in an
outpatient setting as outlined in both Medical Group policy and Wm.
J. Holloway Community Program procedure. Serves as advocate for
patients’ rights and intervenes to promote and defend these in
order to ensure a patient’s safety and well-being. Provides support
to providers, nursing, and allied health caregivers in the
management of admitted patients. Serves as a resource for inpatient
staff and assists in coordinating outpatient management planning to
facilitate patient care and to enhance patient compliance with
discharge plans. Demonstrates awareness of self-learning needs and
seeks ways to meet these in order to maintain current competency
and respond to new clinical and educational demands. Interfaces
with resource people in order to reinforce expert clinical care and
facilitate self-development. Attends appropriate
conference/inservice to maintain current knowledge base
Demonstrates the ability to use equipment in clinical setting
according to nursing procedures. Completes routine competencies for
clinical equipment as per ChristianaCare standards. Contributes to
the professional development of peers by supporting and
role-modeling, precepting, and mentoring. Assists in the clinical
teaching of professional students, both graduate and undergraduate.
Acts as a resource person for all staff and as a direct liaison to
communities throughout the state in educational efforts through
mentoring and clinical precepting. Maintains positive public image
with other employees, patients, and visitors at all times,
demonstrating Christiana Care core values and behaviors. Assists,
when appropriate, with community-based educational efforts.
Participates in appropriate organizational and community-based
committees. Assists in the development and implementation of
appropriate standards of care utilizing current research findings.
Defines professional and personal development goals and reviews
with management team on a regular basis. Participates in
professional organizations. Attainment of certification and
participation in Christiana Care Professional Nurse development
process is encouraged. Complies with mandatory education programs
annually as directed by ChristianaCare standards. Performs assigned
work safely, adhering to established departmental safety rules and
practices. Reports to supervisor, in a timely manner, any unsafe
activities, conditions, hazards, or safety violations that may
cause injury to oneself, other employees, patients and visitors.
Performs other related duties as required. EDUCATION AND EXPERIENCE
REQUIREMENTS: A graduate of a state approved or National League of
Nursing accredited School of Nursing. Two years’ experience as a
RN, physician office experience preferred. BSN preferred; AND
considered with agreement of completion of BSN in 3 years. SPECIAL
REQUIREMENTS: Current RN licensure in the State of Delaware BLS

- $61.16 This pay rate/range represents ChristianaCare’s good faith
and reasonable estimate of compensation at the time of posting. The
actual salary within this range offered to a successful candidate
will depend on individual factors including without limitation
skills, relevant experience, and qualifications as they relate to
