The Population Health Specialist (PHS) position resides within the Department of Population Health Management (PHM), Value Based Services Organization (VBSO). The PHS is responsible for the identification of pediatric population healthcare needs (or needs that are impacting healthcare) based on trends and a variety of data sources. The role, working closely with the QI Specialist, will develop protocols that are evidence based and/or based on best practices. The role focuses on the needs of Nemours' patient population, the clinically integrated network's patient population and the broader pediatric population. The associate will participate in multi-disciplinary teams to ensure the successful implementation of various projects or interventions (examples may include items such as, trauma informed care, school-based wellness centers, preventative health screenings, the community health needs assessment, initiatives addressing social determinants of health, etc.). This will include possessing the ability to create and maintain project documentation for population health interventions/initiatives including charters, plans, value/impact assessment, outcomes and other program documents as needed. The associate coordinates with other PHM roles with tracking and monitoring individual practice process and outcome measures for projects or interventions for which the PHS is accountable for implementing. Finally, the associate will support the practices in non-clinical endeavors/initiatives as identified. This can include, but is not limited to assisting the staff in the initial creation and ongoing maintenance of process procedure and training plan documentation for population health programs. The PHS may also support grant writing and fulfillment of grant requirements, as determined by the department lead. The PHS reports to the department lead, Director of Population Health Management or the VBSO Executive Director. Master's Degree Preferred.
