Job Description:
This role involves providing high-quality hospice care by
collaborating with patients, families, physicians, and the
interdisciplinary team to manage symptoms and ensure appropriate
evaluations and certifications.
Serve as a consultant for acute clinical issues via in-house or
phone consultation, and participate in educational activities to
improve care and promote hospice services.
Act as a member of the Professional Advisory Committee and support
access to hospice care through outreach.
Qualifications include a medical degree, current license, hospital
medical staff membership, reliable transportation, and preferably
board certification in hospice and palliative care.
