Job Description:

Chester County provides supportive services including outreach,
monitoring, assessment and service planning, support network
building, consultation to families, side-by-side skill training and
instruction in the use of community resources, including referrals
to community assistance as needed.
Measures of Performance:
Position Requirements:
Provide service coordination for an assigned group of clients
including coordinating and monitoring the activities of the
individual treatment team (ITT); assume responsibility for
developing, writing, implementing, evaluating, and revising overall
treatment goals in collaboration with the client and team; provide
individual and supportive therapy and symptom management.
Assist clients to find and maintain a safe and affordable place to
live - apartment hunting, finding a roommate, landlord negotiation,
cleaning, furnishing, and decorating, and procuring
necessities.
Assist and support clients to carry out personal hygiene and
grooming tasks.
Provide nutrition education and assistance with meal planning,
grocery shopping, and food preparation.
Ensure clients have adequate financial support (e.g., gain
employment or apply for entitlements).
Teach money management skills and assist clients in accessing
financial services.
Assist clients with accessing reliable transportation.
Provide side-by-side support and coaching to help clients socialize
(e.g, going with a client to a basketball game).
Position Requirements Continued
Plan, structure, and prompt social and leisure-time activities on
evenings, weekends, and holidays.
Participate in client-centered comprehensive assessment of
psychiatric history, mental status, physical health, use of drugs
or alcohol, education and employment, social development and
functioning, activities of daily living, and family structure and
relationships.
Provide on-call crisis interventions and serve as back up after
hours on rotating schedule.
Maintain a valid driver's license and satisfactory driving record
in accordance with St. Luke's policy.
Requirements:
A Bachelor's degree with major course work in criminal justice
sociology, social work, psychology, counseling or education; and
two years direct service experience with mental health.
Understanding of principles of mental illness and ability to
implement them, and behavioral best practice and crisis management
planning.Valid driver's license with driving record in accordance
with St. Luke's policy.
